For New and Established Patients to Schedule or Re-Schedule an appointment. If it's less than 24 hours before your scheduled appointment time and you need to reschedule, please text the office at 816-407-7200. 

If you would like to find out how much a visit may cost, click here for a Quote of Benefits request and our billing department will verify your insurance and provide you an emailed quote. If no insurance, ask for a Self-Pay Quote.

To request a copy of a receipt or itemized statement, please email the office your Name, Date of Birth and current email address and exactly what you are needing and we will email you back as soon as we can.

Too reach our Office Manager, Elaine Hutchison, regarding a sales call or other issue, please click to email her directly. This is the easiest way to get in touch with her as she does not typically work in the office. 

If you are from an Insurance Company, the VA, Attorney's office or other Provider's office regarding a patient's care, or have a records request, please email the Office Manager by clicking button or fax your request to: 816-407-7222.
